What does high VLDL cholesterol mean?
The short answer
VLDL cholesterol is the cholesterol carried in very-low-density lipoproteins, the triglyceride-rich particles your liver releases into the blood. Most lab reports do not measure it directly: they estimate it as your triglycerides divided by 5. A result of 30 mg/dL or above is usually counted as high, almost always reflects high triglycerides, and is linked with higher heart disease and stroke risk.
What VLDL measures
VLDL (very-low-density lipoprotein) is a particle your liver makes and releases into the bloodstream. It is mostly triglyceride, with some cholesterol and a protein called apolipoprotein B. As VLDL hands its triglyceride to the body's tissues, it shrinks into cholesterol-rich remnant particles and LDL.
On most lab reports, including the Wolfburgh report, VLDL is a calculated value rather than a separate measurement: your triglyceride result divided by 5. This is the same VLDL term used in the Friedewald formula that calculates LDL (total cholesterol minus HDL minus triglycerides divided by 5).
The estimate assumes a fasting sample in which VLDL carries roughly one part cholesterol for every five parts triglyceride. That holds reasonably well at normal triglyceride levels and breaks down as triglycerides climb or soon after a meal.
VLDL ranges
There is no guideline target for VLDL itself. These bands are the reference values published by MedlinePlus (US National Library of Medicine) and Cleveland Clinic.
| Result | Range |
|---|---|
| Normal (MedlinePlus) | 2 to 30 mg/dL |
| Normal (Cleveland Clinic) | Below 30 mg/dL |
| High (Cleveland Clinic)Usually goes with triglycerides of 150 mg/dL or more, since 30 x 5 = 150 | 30 mg/dL or above |
| Estimate less accurateWolska and Remaley, NIH | Triglycerides above about 250 mg/dL |
| Estimate not reliableMedlinePlus | Triglycerides above 400 mg/dL |
Because VLDL is triglycerides divided by 5, the top of the normal VLDL range (30 mg/dL) lines up with the top of the healthy triglyceride range (below 150 mg/dL, NHLBI). Reference ranges vary between laboratories, and the range printed on your own report takes priority. Ranges vary between laboratories, and the range printed on your own report takes priority.
What a high VLDL result means
A high VLDL almost always means your triglycerides are high, because on the report one is calculated from the other. MedlinePlus notes that a high VLDL may be associated with a higher risk of heart disease and stroke. The reason is what VLDL becomes: as the particles release triglyceride they turn into cholesterol-rich remnants and LDL, which can build up in artery walls.
Doctors rarely treat VLDL as a target in its own right. LDL is usually the main treatment target, and VLDL is read alongside your triglycerides, LDL and HDL. Cleveland Clinic notes that a high VLDL together with a high LDL is more concerning than either alone.
Common causes
- Eating a lot of foods high in fat and sugar
- Drinking too much alcohol
- Overweight or obesity
- Not getting enough physical activity
- Diabetes, especially when poorly controlled
- Thyroid, kidney or liver disease
- Some medicines, including some used for breast cancer, HIV and high blood pressure
- Inherited disorders of fat metabolism
What a low VLDL result means
Neither MedlinePlus nor Cleveland Clinic sets a lower limit of concern for VLDL. A low VLDL simply mirrors a low triglyceride result, and MedlinePlus lists the possible causes of that below. TSH, which is on the same panel, helps your doctor check the thyroid side of that list.
Common causes
- A low-fat diet
- An overactive thyroid (hyperthyroidism)
- Malabsorption, where the small intestine does not absorb fat well
- Malnutrition
What moves VLDL
Nutrition
VLDL tracks triglycerides, so the dietary levers are the triglyceride ones. The NHLBI lists foods high in fat and sugar as a cause of high triglycerides and recommends heart-healthy choices; Cleveland Clinic suggests a Mediterranean-style diet.
Training
Regular physical activity is one of the NHLBI's core steps for lowering triglycerides, and Cleveland Clinic lists exercise as a way to lower VLDL. Check with your doctor first if you have a heart condition.
Sleep and stress
We have not found strong direct evidence that sleep or stress changes VLDL on its own. Where they drive late eating, alcohol or missed training, those habits are the practical target.
Habits
Too much alcohol raises triglycerides, and the NHLBI notes a provider may recommend stopping alcohol when levels are very high. Reaching a healthy weight is the other major lever.
How fast VLDL can change
US guidance repeats a lipid test 4 to 12 weeks after starting or adjusting cholesterol medication, to check the response to medication and lifestyle change (2018 ACC/AHA guideline, summarised in American Family Physician). A Day 90 re-test falls just after that window, so it is a reasonable point to see whether sustained changes have moved your triglycerides, and with them your VLDL. Compare results taken under the same conditions.
Before your test
- Follow the fasting instructions you were given. MedlinePlus advises not eating for 8 to 12 hours before a triglyceride test, and a non-fasting sample makes the VLDL calculation less reliable.
- Avoid alcohol beforehand unless told otherwise; MedlinePlus notes alcohol can interfere with triglyceride results.
- Above 400 mg/dL of triglycerides the VLDL figure is not reliable, and accuracy already falls above about 250 mg/dL.
- Pregnancy can affect triglyceride results, and therefore VLDL (MedlinePlus).
When to speak to your doctor
Speak to your doctor if your VLDL is above the range on your report, especially alongside a high LDL or other risk factors such as diabetes, high blood pressure, smoking or a family history of early heart disease. A very high result needs prompt attention: the NHLBI notes that triglycerides above 500 mg/dL raise the risk of acute pancreatitis, a sudden swelling of the pancreas. Seek urgent medical care for severe abdominal pain.
On the Wolfburgh panel
VLDL Cholesterol is one of the 17 markers on the Wolfburgh blood panel. The lab calculates it from the measured markers on the same report, so it costs nothing extra and needs no extra sample. Your sample is collected at home with RedDrop One, an FDA-cleared collection device, and a licensed physician reviews every panel before your care team sees it.
VLDL is calculated from your triglyceride result, so the Nutritionist and Trainer work on the levers that move triglycerides (sugar, alcohol, weight and regular activity), with the Behaviour Specialist helping the changes stick. Your Partner Physician reads VLDL alongside LDL, HDL and HbA1c, since diabetes is a common driver of high triglycerides. Members re-test at Day 90, backed by the 90-Day Results Guarantee.
Common questions about VLDL
What is a normal VLDL level?
MedlinePlus gives a normal VLDL cholesterol range of 2 to 30 mg/dL, and Cleveland Clinic treats 30 mg/dL or above as high. Because VLDL is usually triglycerides divided by 5, a VLDL of 30 mg/dL matches triglycerides of 150 mg/dL, where the NHLBI's healthy triglyceride range ends. Use the range printed on your own report.
How is VLDL calculated on a blood test?
Most labs estimate VLDL as your triglyceride result divided by 5, which assumes a fasting sample in which VLDL carries about one part cholesterol for every five parts triglyceride. The estimate becomes less accurate as triglycerides rise above about 250 mg/dL, is not reliable above 400 mg/dL, and is less reliable on a non-fasting sample.
How can I lower my VLDL?
Lower your triglycerides, because VLDL is calculated from them. The NHLBI's main lifestyle steps are heart-healthy food choices with less fat and sugar, regular physical activity, reaching a healthy weight and cutting back on alcohol. If you take cholesterol or triglyceride medication, keep taking it as prescribed and discuss any changes with your doctor.
Is VLDL worse than LDL?
They are stages of the same system rather than rivals. As VLDL particles release triglyceride they turn into remnants and LDL, and both can contribute to plaque in artery walls. Doctors usually treat LDL as the main target and read VLDL as a pointer to triglycerides. A high VLDL with a high LDL is more concerning than either alone.
